A Prescription

Your original order must be accompanied by a current prescription from your doctor. After this initial order, we will retain your prescription on file for future reference, until the expiration date of the Rx.

Your prescription can be obtained by simply visiting or calling your doctor's office and requesting a copy. In most states, the doctor is required by law to provide you with your prescription upon your request.

At the minimum, your prescription must include: the base curve, power, diameter and specific lens material for each lens. Special lens features, if any, should be clearly defined.

Contact lenses provided by RGP Lens Company are for the exclusive use of the specific person named in the prescription that is being filled.

Order Form

You can fill out the order form online then print it, or save it in a file for E-mail attachment.

Both documents, prescription and order form, are then FAXED or E-mailed to us to initiate the order.

Payment

All orders must be paid through PayPal or Money Order. See the detailed procedure for PayPal and Money Order Payment Options on the 'Placing Your Orders' page.

For your security, we do not accept personal checks.
